HOW TO MAKE A REFERRAL
Do you have a patient who may benefit from hospice services? As health care professionals, we are all dedicated to the same mission - providing care that improves quality of life and preserves human dignity. We are dedicated to working with you, your patients and their families to deliver clinically excellent, compassionate, customized care.
Call us when your patient:
- could reach the end of life within a year
- is facing difficult care decisions
- has a serious illness and is suffering from pain or other symptoms
- is being treated for a chronic condition and needs additional nursing support, counseling or other customized care
- calls more often or is being admitted to the emergency department/hospital more often
- experiences challenges providing care for a loved one or friend
- lives in Yakima County
Our services for your patient includes:
- 24-hour nursing availability
- Care in any setting - home, hospital, assisted living and nursing facilities
- Direct admission to facilitate hospital discharge planning
- Access to pharmacy, durable medical equipment, medical supplies, infusion, skilled therapies

Please be able to provide us with the patient's demographic information (birth date, address, phone) as well as names of their next of kin/contacts, and their diagnosis with a brief explanation of why this patient needs hospice care (symptoms, pain, areas of suffering).
