Referrals

How does a patient or family get a referral?

Anyone is welcome to call Memorial Hospice for more information about hospice for themselves or a loved one. To receive hospice services, a physician must make a referral.

You don't need a doctor's referral to contact us; we'll arrange for the physician's order approving Hospice care so we can begin to care for you or your loved one as soon as possible.

When should someone seek hospice?

  • There is a diagnosis of an illness which will result in a limited life expectancy (usually six months or less).
  • Curative measures are not being sought, although a patient or their family may change their mind at any time to pursue active treatment.
  • There is concern about managing care in the future, and the family wants to explore long-term options.
  • There is a need for comfort care, pain and symptom management.
